Please take the time to cancel any appointment you will not be able to attend as soon as you can, preferably 1 day prior to the appointment time. Any appointment missed, late cancelled, or changed without 24hour notice will result in a charge equal to 100% of the reserved service.

Dear client,

the protection of your personal data is very important to me. According to the European Basic Regulation concerning personal data (DSGVO), it is my obligation to inform you about the purpose of collection, distribution and aggregation of data within the shiatsu office. You can also find information on your rights concerning personal data (see also "Impressum").

2. COLLECTION, PROCESSING AGGRAVATION AND DISTRIBUTION OF DATA

The collection, processing, aggravation, and distribution of data is effected in accordance to legal requirements, in order to guarantee the contractual performance between you and your Shiatsu Practitioner and the associated rights and obligations.

For this purpose, we process your personal data, especially those concerning your health. This includes, anamnesises, findings, treatment proposals, and those to support client care at home of me or doctors. For this purpose, it is also possible, that your treating doctors or psychotherapists can provide treatment and patient data (e.g. physician's letters).

The collection of health data is prerequisite for your treatment.

If you don't keep me informed or necessary information is withheld or, I cannot provide meticulous treatment according to your individual specific needs.

3. RECEIVER OF DATA

We only pass on your relevant data to others, as far as it is permitted by law or with your express agreement.

Receivers of your personal data receivers can be therapists in particular. The transfer of personal data serves billing purposes according to treatment provided for you.

4. RETENTION OF DATA

We only keep personal data for the time necessary to guarantee adequate treatment for you.

Due to legal requirements, we are obligated to retain your personal data for a period of 10 years, at least, after completion of treatment. Due to other provisions, longer retention periods may apply.

5. YOUR RIGHTS

It is your right to receive information about the personal data concerning yourself and you can request inaccurate data to be corrected, of course.

Apart from that it is your right to demand deletion of your personal data under certain conditions, as well as to limit the processing of data and its transfer.

The processing of data takes place in accordance to legal regulation. Please note, that only in special cases, we require your consent. In this case, you have the right to withdraw your consent for the processing or traffic of data for the future.

Furthermore, you have the right to make a complain at the regulating authority for data privacy, if of the opinion that the processing of personal data does not follow legal regulation.

6. LEGAL FOUNDATION

The legal foundation for the processing of personal data is Article 9(2) lit. h) DSGVO in conjunction with § 22 paragraph 1 No. 1 lit. b) Federal Data Protection Act. In case of questions, please feel free to contact us.

7. FURTHER NOTES ON COURSE REGISTRATION VIA "KurVe" PARTICIPANT MANAGEMENT (Kidsgo) 

KurVe is a web-based software for participant management, developed by kidsgo Verlag GmbH (kidsgo) in consultation with users. Data processing as according to the principals of orderly processing of data and legal regulations.

By and through registration, users consent to automatic processing and storage of all personal data entered. This can be:

a) participant related data: surname, name, postal address, mail-address, phone number etc.

b) course related data: date of birth of the child, age of the child, name/s of siblings, terms of payment etc.

Utmost care is taken, that only strictly necessary data concerning course participation are collected. There will be no disadvantage to participants, who do not fill in empty fields with voluntary data (such fields are not marked with *). Data will only be stored until necessary in order to handle the course management and that of following-up courses properly, if appropriate and due to legal requirements.

The processing of personal data may be revoked at any time. Herefore please don't hesitate to contact:

It is your right according to § 15 DSGVO to demand complete information at any time concerning your personal data (purpose, storage, processing, transfer etc.) from Susanne Pitruzzella.

According to §17 DSGVO it is your right to request deletion, blocking, and withdraw authorisation of data processing consent concerning singular data from Mrs Susanne Pitruzzella, at any time.

Furthermore, you may - without giving reasons - exercise your right of opposition and completely withdraw or alter your Written Declaration of Consent for future purposes via letter, phone, E-mail, at any time. Thereby, no additional costs apart from postage and/or basic tariff for transmission will arise for you.
